Our Person Centred Goals

The way in which our goals are set makes them truly person centred – it is often easier for us to do this as many of our clients are living with their disabilities in the community and have had time to discover what would really make a difference to their lives.

Setting the Goals

Assessment is a collaborative process, our clients ultimately choose which areas they would like to address and our therapists discuss the possibilities openly with them. We will give information on other areas we feel could be improved upon but the choice of goals to be worked toward is firmly in the hands of our clients.

David’s Goals

David is a 56 year old man who was brain injured in a road traffic accident and was referred by his case manager to our team. David’s 2 main priorities were to improve his walking and to drive a car again. In addition our physiotherapist and occupational therapist felt he had good potential to improve his right arm function. We informed David of our findings and he chose not to work on his arm but to focus on his main goals.

With therapy input David improved his walking to the point where he no longer needed his walking stick and could walk faster and further, we explored the issues of concern regarding his driving with the use of appropriate computer software, a visit to a go-kart track and onward referral for appropriate visual testing and driving assessment at his local driving centre.

A year later David returned to us to ask us to work with him on his right arm – he felt that having achieved his original goals he was ready to work on this area.

Sarah’s Sustainable Success

Sarah has achieved sustainable change, which had been difficult to attain in the 4 years since her cerebral tumour removal. She required fairly intensive therapy but lived out of the area and many of the things she needed to improve were tasks around her home.

We were able to arrange for Sarah to spend her respite at Chase Park Rehabilitation Centre where we could intensively provide her with a full package of therapy whilst her family had a break. After 2 weeks of intensive therapy where she made many positive changes in her abilities she returned home and was able to continue making changes and using the new skills she had gained back in her own environment. Careful communication with her family and local therapists meant that she achieved excellent ‘carryover’ of her new skills.

Sarah has since returned for 5 further short bursts of rehabilitation at Chase Park Rehabilitation Centre over a 2-year period and has been able to put into practice what she has gained at home. She has maintained her new skills well. We think this really is ‘The Best of Both’ – the chance to have an intensive ‘blast’ of rehab but to take the improvements back home to consolidate what has been learned - an effective example of creating sustainability.

Economy

Whether our services are funded by the individual, a legal claim or by the NHS we believe that providing a quality service which holds meaning for people and creates sustainable change offers value for money.

Long Term Cost Savings

We have many rehabilitation examples where our clients have been able to improve to a point where less support is needed, meaning that the cost of care is reduced – a very significant saving for the state or perhaps an insurance company (in the case of personal injury claims).

‘Becoming a Contributor Again’

We work closely with vocational training organisations, which may be the next step for many of our clients so that people can progress to paid or voluntary employment as well as taking up university or college courses and so re-entering society and ‘becoming a contributor again’ as Joe put it when he was able to achieve part time paid employment.
